We are passionate about Georgians completing their advance care plans, and we strongly urge Georgians to complete the Georgia Advance Directive for Healthcare.

The Georgia Advance Directive, however, is not the only advance care planning document available.

In our last blog, we explored Five Wishes. Today we’re exploring PREPARE for Your Care

PREPARE for Your Care

This online resource provides information and guides to help individuals prepare for medical decision-making. It is easy to use and is filled with video stories and examples.

In an effort to reduce any barriers to advance care planning, especially health literacy, PREPARE for Your Care created an accessible tool that is free and evidence-based. Knowing that most advance directives are written in difficult-to-understand legal language, PREPARE for Your Care is written at a fifth-grade reading level. It is available in English and Spanish.

The PREPARE website includes:

A video introducing why and how people can use PREPARE;
The PREPARE pamphlet, which provides an easy way to introduce patients to advance care planning;
The PREPARE Toolkit, which helps community groups set up and run a PREPARE session; and
Access to an easy-to-read Advance Directive.

Using a five-step process, the website covers how to choose a surrogate, how to start these conversations, how to ask your doctors questions.

The website is free to use, and the advance directive can be downloaded for free on their website.
