Help Families Navigate Aging with Compassion and Confidence

We are seeking a Social Worker to join our interdisciplinary team in our Vinings office. Working alongside Certified Elder Law Attorneys, public benefits specialists, and our practice management team, you will help older adults and their families navigate healthcare, long-term care, and aging-related challenges with confidence and dignity.

This position reports to the firm’s Care Coordination leadership and offers a unique opportunity to combine social work, advocacy, care coordination, and relationship building in a collaborative legal setting.

 

What You’ll Do

Client Advocacy & Care Coordination

As a key member of our Care Coordination team, you will:

  • Serve as the client’s advocate and care specialist on an interdisciplinary legal team.
  • Serve as the primary point of contact for assigned clients and their families.
  • Build trusted relationships with clients, caregivers, healthcare providers, and community partners.
  • Assess clients’ independence, safety, and ability to care for themselves.
  • Develop individualized care plans and provide ongoing telephone follow-up at least quarterly, or more often as needed.
  • Educate clients and families on aging issues, home care, long-term care, placement options, community resources, and care costs.
  • Help families navigate the healthcare and long-term care systems and identify suitable long-term care placements.
  • Coordinate appropriate care services, including home care, rehabilitation, assisted living, memory care, skilled nursing, and other long-term care options.
  • Advocate for clients to receive quality healthcare and long-term care services.
  • Provide compassionate guidance and support during family crises.
  • Assess clients’ decision-making capacity when appropriate.
  • Coordinate the timely execution of legal documents for assigned clients.

 

Team Collaboration & Professional Engagement

You will also:

  • Participate in client meetings and interdisciplinary care planning discussions.
  • Communicate changes in client status with attorneys and other team members.
  • Maintain accurate documentation within the firm’s client management system.
  • Develop a working knowledge of all practice areas within the firm.
  • Support fellow team members and assist with office needs as appropriate.
  • Build and maintain strong relationships with hospitals, rehabilitation centers, senior living communities, home care agencies, and other healthcare professionals.
  • Participate in community education programs, workshops, presentations, and professional networking events as needed.

Qualifications

We’re looking for someone who is compassionate, organized, collaborative, and passionate about serving older adults and their families.

Required Qualifications

  • Degree in Social Work.
  • Current Georgia Social Work license or actively working toward licensure.
  • Excellent written, verbal, and interpersonal communication skills.
  • Strong organizational, planning, and time management abilities.
  • Ability to work independently while thriving as part of a collaborative team.
  • Excellent judgment, professionalism, and ability to maintain confidentiality.
  • Strong computer skills, including Microsoft Office, email, client management software, and Apple devices (Mac and iPhone).
  • Valid Georgia driver’s license, reliable transportation, and proof of insurance.
  • Willingness to travel locally (up to approximately 100 miles from the office) to visit clients as needed.
