We are passionate about Georgians completing their advance care plans, and we strongly urge Georgians to complete the Georgia Advance Directive for Healthcare.

The Georgia Advance Directive, however, is not the only advance care planning document available.

In previous blogs, we have explored, Five Wishes and PREPARE for Your Care.

Today’s blog is all about Georgia’s own Critical Conditions.

Critical Conditions

Imagine you are lying in a hospital bed unconscious and not able to speak for yourself. Decisions need to be made about medical treatments to prolong your life. How would your loved ones feel about having to make these decisions? Would they know what you want? Those who have faced this situation can tell you how hard it is. They will also tell you how much they wish they had known what their loved one wanted. Don’t let this happen to your family. Talk to them about your final health care choices.”

This is how the CRITICAL Conditions℠ Planning Guide starts. And we couldn’t agree more.

The CRITICAL Conditions℠ Planning Guide℠ was developed by Georgia Health Decisions to help families make difficult decisions, have hard conversations, and prepare an advance directive. It offers a step-by-step guide in a booklet that can be downloaded from the Emory Center for Ethics and the Healthcare Ethics Consortium’s website.

The guide offers conversation starters, explains various medical procedures and includes a Georgia Advance Directive form.

Since Critical Conditions provides the Georgia statutory form of the Advance Directive, it is in compliance with Georgia code.
