Medicare Advantage enrollment is declining

Over the past two weeks, we have shared blogs with Medicare open enrollment information and advice. Each year during the fall, we have countless questions from our clients about which plan they should choose. Medicare Advantage enrollment is declining

Medicare Advantage (MA) plans are enticing to some because of their often-lower premiums and some added ‘perks’. Starting this year, an interesting trend is the decline in the number of people choosing MA plans.

The trend for 2026 signals a pivotal shift for Medicare Advantage enrollment. For the first time in nearly two decades, the Centers for Medicare & Medicaid Services (CMS) projects a decline, from about 34.9 million in 2025 to around 34 million in 2026 which lowers MA’s share of Medicare beneficiaries from approximately 50% to 48%.

What’s happening with Medicare Advantage?

Several key factors drive this change. Insurers are withdrawing from certain MA markets and cutting back on unprofitable plans due to tighter margins and rising healthcare costs. Meanwhile, the J.D. Power 2025 U.S. Medicare Advantage Study   shows a 29-point decrease in member satisfaction (on a 1,000-point scale), caused by a 39-point drop in trust toward plans which is linked to increased confusion over prior authorizations, narrower networks, and shifting benefit structures.
